The other problem I found in the old days (non-Agile) was the opposite: I got empowered managers providing the information for specs which didn't reflect what the people using the software were doing. The only solution was to insist on shadowing unempowered workers whose work patterns let you know, e.g. the order of steps they took to build a transaction.

Hierarchies tend to end up with a situation where power and knowledge are separated, except in the very rare case of someone who really rose up through the ranks - and even then their knowledge gets out of date.
