Whence the long list of required sign-offs on every requirements spec I ever saw. By the time everybody has been consulted, and the document circulated, and acceptances collated, responsibility is so blurred it might as well not exist.
And that assumes that the assembly of information was being done by someone otherwise competent. In my experience this applies to a few but not many Analysts.
